Content Guru and Simply Connect have been awarded places on the new 2021 NHS England Health Systems Support Framework (HSSF) for social prescribing, which itself forms part of NHS England’s national strategy to move to an integrated care model.
Social prescribing, an initiative to help those dealing with loneliness and financial struggles, allows healthcare workers to signpost patients for community support, with the aim to improve health and well-being.
The HSSF provides a streamlined way for NHS England organisations to procure services from approved suppliers with solutions that enable digital and service transformation. The Framework focuses on services that can support the move to integrated models of care based on intelligence-led population health management.
To help implement social prescribing, Content Guru and Simply Connect work together to offer a combined solution that allows GPs, healthcare professionals and support agencies to refer individuals to social prescription link workers, or directly to validated community services.
Content Guru storm, the digital social prescribing solution for cloud communication services, offers a public online portal for personalised social prescribing, detailed case management and health monitoring, results reporting, and self-introduction, the two firms claim.
